The information provided in the Application for Employment is true, correct, and complete. If employed by Western-Shamrock Corporation, any misstatement or omission of fact on this application may result in my dismissal. I understand that acceptance of an offer of employment does not create a contractual obligation upon the employer to continue to employ me in the future. I agree that the Company may obtain a Motor Vehicle Report (MVR) on me. Results may cause dismissal/no hire. I authorize you to conduct a criminal background check. If you decide to engage an investigative consumer-reporting agency to report on my credit and personal history, I authorize you to do so. If a report is obtained you must provide, at my request, the name of the agency so I may obtain from them the nature and substance of the information contained in the report. I authorize you to verify my employment with employers listed on this application.

I voluntarily and knowingly authorize any present or past employer or supervisor, college or university or other institution of learning, administrator, law enforcement agency, state agency, local agency, federal agency, credit bureau, collection agency, private business, military branch or the national personnel records center, personal or professional reference, and/or any other person to give records of information that may have concerning my criminal history, motor vehicle history, social security number, earnings history, character, employment (including reasons for termination), credit history, credit capacity, credit standing or any other information requested by Western-Shamrock Corporation deemed pertinent to my employment.

Information obtained may be used in employment decisions including termination.

In considering you for employment and, if you are employed, in considering you for subsequent promotion, retention, or discipline, WESTERN-SHAMROCK CORP. (“the Company”) may request and rely upon one or more consumer reports and/or investigative consumer reports about you that we obtain from a consumer reporting agency, such as Western Verify and/or CSC Credit Services. These reports may contain information about you as to your character, general reputation, personal characteristics, and mode of living.

Under the FCRA, before the Company can obtain a consumer report or investigative consumer report about you for employment purposes, we must have your written authorization. Before we take adverse action on the basis, in whole or in part, of information in that report, you will be provided a copy of that report, the name, address, and telephone number of the consumer reporting agency, and a summary of your rights under the FCRA.

AUTHORIZATION

Pursuant to the Federal Fair Credit Reporting Act, I hereby authorize WESTERN-SHAMROCK CORP. and its designated agents and representatives to conduct a comprehensive review of my background through a consumer report and/or an investigative consumer report to be generated for employment, promotion, retention, or discipline as an employee. I understand that the scope of the consumer report/investigative consumer report may include, but is not limited to, the following areas: verification of Social Security number; current and previous residences; employment history, including all personnel files; education; references; credit history and reports; criminal history, including records from any criminal justice agency in any or all federal, state or county jurisdictions; birth records; motor vehicle records, including traffic citations and registration; and any other public records.

I authorize the complete release of these records or data pertaining to me that an individual, company, firm, corporation, or public agency may have. I hereby authorize and request any present or former employer, school, police department, financial institution, or other persons having personal knowledge of me to furnish WESTERN-SHAMROCK CORP. or its designated agents with any and all information in their possession regarding me in connection with an application of employment. I am authorizing that a photocopy of this authorization be accepted with the same authority as the original. I understand that, pursuant to the Federal Fair Credit Reporting Act, if any adverse action is to be taken based upon the consumer report, a copy of the report and a summary of the consumer’s rights will be provided to me.

I agree that this authorization shall remain valid for the duration of my employment with the Company. I certify that the information contained on this Authorization form is true and correct and that my application may be terminated based on any false, omitted, or fraudulent information.
